Representing many carriers, Sure Health Choice offers a wide array of plans to choose from. We have local agents who are available to meet with you in person or assist with a contactless enrollment. Our flexibility allows us to meet each client’s needs. Our service doesn’t end with the sale. We pride ourselves on servicing all our clients throughout the entire Medicare process. We are available to answer questions and review policies annually to ensure your policy is still the right one for you. Medicare Supplement insurance is available to those age 65 and older enrolled in Medicare Parts A and B and, in some states, to those under age 65 eligible for Medicare due to disability or End-Stage Renal disease.

Medicare Supplement insurance is available to those age 65 and older enrolled in Medicare Parts A and B, and in some states to those under age 65 who are eligible for Medicare due to disability, ALS (Amyotrophic Lateral Sclerosis, also known as Lou Gehrig’s disease), or End Stage Renal Disease. Not connected with or endorsed by the United States Government or the Federal Medicare Program.

The Centers for Medicare and Medicaid Services (CMS) does not review or approve Medicare Supplement plan information. Medicare Supplement plans cannot be held with Medicare Advantage plans.

We do not offer every plan available in your area. Currently we represent 11 organizations which offer 2,200 products in your area. Please contact Medicare.gov, 1-800-MEDICARE, or your local State Health Insurance Program (SHIP) to get information on all of your options.

Submitting this form will not impact your current Medicare Part A and Part B enrollment, nor will it enroll you in any Medicare plan.

For plans with Part D Coverage: You may be able to get Extra Help to pay for your prescription drug premiums and costs. You can also contact the Social Security Office at +1 (800) 772-1213 between 7 a.m. and 7 p.m., Monday through Friday (TTY: +1 (800) 325-0778 or www.socialsecurity.gov), or your Medicaid office.

You must have both Part A and Part B to enroll in a Medicare Advantage plan. Members may enroll in the plan only during specific times of the year. Contact the plan for more information.
